Your first visit, step by step

A first visit takes about an hour.

  1. Step 1: Request a visit

Use the form or call us. Tell us the day that suits you and how you would like us to reply.

  2. Step 2: We check your benefits

If you have dental insurance, we check your plan's benefits and tell you what it says it covers before you come in.

  3. Step 3: Fill in your forms

Health history and consent forms live in the practice's own secure portal, not on this website. Paper forms are there if you would rather.

  4. Step 4: Your exam

Your dentist examines your teeth, gums and jaw, takes X-rays if they decide you need them, and talks through what they found.

  5. Step 5: Your questions

Ask about materials, snoring and sleep, or a sore jaw. We answer what dentistry can answer and tell you when to see your physician.

  6. Step 6: Your plan in writing

You leave with your options in writing, and an estimate when you ask.

Fill in your new-patient forms

Before you come in

What do I bring to the first visit?

Your insurance card if you have one, a photo ID and a list of the medicines you take. Bring recent dental X-rays if you have them, or ask your old office to send them.

Do I fill in medical details on this website?

No. Our contact form asks for no medical details. Your health history goes on the forms in the practice's own secure portal.

Can you get my records from my old dentist?

Yes. Give us your previous practice's contact information and we will request your records, with your written permission, before your first visit.

Can I bring a sleep study or a doctor's note?

Yes, if you have one. Bring it, or ask your physician to send it, and we will read it before we talk about an appliance.

Ready to book a first visit?

Tell us a good time and how to reach you. We'll take it from there.
